The fenestration is completely clotted over and the cardiologists aren't able to get through to inspect the conduit. Riley will be down from the cath lab with a catheter still in place. Over the next 12-24 hours they'll drop tPA into the catheter and directly onto the fenestration clot. One of three things could happen:
- The clot will dissolve and the fenestration will flow freely. We should see his oxygen saturations drop if this is the case (don't hold your breath).
- The clot dissolves but the doctors don't like the look of the fenestration conduit, possibly because it's too narrow, kinked or whatever. Depending on what they find they could attempt catheter or surgical intervention, everything from stenting to a new fenestration.
- The tPA is unable to remove the clot and another course of action will need to be considered, possibly a new fenestration in the cath lab or a surgical re-do.

PHEW! What a day this has been for all of you. Hopefully you all get some good rest tonight while Riley sleeps off his sedation. Still hoping for the best case scenario. Hugs for the boys and warm thoughts.

Thanks for the update Ken. We're hoping and praying that the clot was caused by something that is correctable in the cath lab tomorrow. And we're hoping that the sats start dropping overnight. It's about time things started going right for you guys.
I've seen a lot of frustration in these comments since last night - I think it's because we all feel that you deserve a break, something to go right, without complication or qualification. We are so frustrated for you guys. You have a wonderful family, great doctors, an incredibly strong and brave little boy... it's time for your luck to change for the better.

Unfortunately there isn't going to be a lot of oneline data that is going to be found. Its hard and frustrating for those who want to educate themselves about all the ins and outs of what is happening and what may happen.
All these procedures and specifically their application in this scenario are fairly rare(as opposed to your "run-of-the-mill" coronary artery bypasses and stenting).
These things happening to Riley are Very rare and there are a handfull of surgeons experienced in this specialized area.
Its got to be frustrating for everyone.
The current coarse really seems to be the only coarse based upon what is being said.
So once again, we sit and wait.
Greatful for where we are, considering where we could be.
Riley is stable and that does mean a lot in the big picture for now.
It doesn't make it any easier though.

I am hoping and praying for option number one! I am also praying for the strength and courage for you to get over yet another hurdle.
Like everyone else, I can't believe everything you guys are having to go through. I know how it feels to have a child in the hospital with seemingly no end in sight. You are in our thoughts and prayers constantly. I'll be checking in frequently and, of course, hoping for the very best for little Riley.
